Update: Twiggy's vent is looking a LOT better after taking out the perch covers. She's also starting to step up on my hand again.

Occasionally scratches at her vent (mostly to groom), but there's little to no visible poop around it last time I checked. Her feathers seem healthier around the vent as well after the whole thing. I haven't seen any poop stuck recently in the feathers around her vent as well, which is good. I haven't had any other problems with Twiggy as of late.
Glad she is improved. I have never liked any kind of perch that is supposed to keep the nails trim. There is a price tag to using them and the expense of the bird.

I remember seeing perches that were gritty and was thinking about getting them. Knowing what I know now, I'll make sure to stay away from them.
